Self—employed private doctors who charge a fee each patient’s visit are the foundation of medical practice in the United States. Most doctors have contract relationship with one or more hospitals in the community. They send their patients to this hospital，which usually charges patients according to the number of days they stay and the facilities (operation room, tests, medicines that they use). Some hospitals belong to a city, a state or a government agency. Others are operated by religious orders (教会)or other non—profit groups.
Some medical doctors are on salary. Salaried doctors may work as hospital staff (员工)members, or residents, who are often still in training . They may teach in medical schools, or be hired by corporations to care for their workers to work for the government’s Public Health Service.
Doctors are among the best paid professionals in the United States. In the 1980s, it was not uncommon for medical doctors to earn incomes of more than $100,000 a year. Specialists, particularly surgeons, might earn several times that amount. Doctors list many reasons why they deserve to be so well rewarded for their work. One reason is the long and expensive preparation required to become a doctor in the United States. Most would—be doctors first attend college for four years, which can cost nearly $20,000 a year at one of the best private institutions. Then they attend medical school for four years. Tuition(学费)alone can more than $10,000 a year. By the time they have obtained their medical degrees, many young doctors are deeply in debt. They still face three to five years of residency(实习阶段) in hospital, the first year as an assistant. The hours are long and the pay is relatively low.
Setting up a medical practice is expensive, too. Sometimes several doctors will decide to establish a group practice, so they can share the expense of maintaining an office and buying equipment. These doctors also take care of each other’s patients in emergencies.
Doctors work long hours and must accept a great deal of responsibility. Many medical procedures, even quite practiced many times, still involve risk. It is understandable that doctors want to be well—paid for making decisions which can mean the difference between life and death.

In March we published a photo story about the extraordinary Zsnskar region in northern India，which is cut off from the outside world for more than seven months of the year，and only accessible (可通行的)via a frozen river．We also included information about the limited use of skiing in the region and the recent creation of the Zanskar Ski School:

“Despite the difficulties of travelling through the region when the snow comes，skiing hasn’t traditionally been used as a means of transport by the locals，largely because trees don't grow here, so there is little in the way of raw materials from which to make skis．In 1995, a group of British scientists in the region noticed the lack of skis and one of them returned to set up the Zanskar Ski School in Padam．The school provides lessons for a small fee and rents skis to the local people. Among the benefits that the school hopes to bring are improved education—children often find it difficult to get to school through the deep snow—and the possibility of offering ski tours to tourists in the future．So far，more than 300 local people have received training，and local doctors and policemen regularly borrow skis.”

A study into how lazy British people are has found more than half of adults are so lazy they'd catch the lift rather than climb two flights of stairs．
Just over 2000 people were quizzed by independent researchers at Nuffield Health, Britain's largest health charity．The results were astonishing．
About one in six people surveyed said if their remote control was broken, they would continue watching the same channel rather than get up．
More than one third of those questioned said they would not run to catch a bus．Worryingly, of the 654 respondents with children, 64% said they were often too tired to play with them．
This led the report to conclude that it's no wonder that one in six children in the UK are classified as obese （very fat） before they start school．
Dr Sarah Dauncey, medical director of Nuffield Health, said: "People need to get fitter, not just for their own sake, but for the sake of their families, friends and evidently their pets too．
"If we don't start to take control of this problem, a whole generation will become too unfit to perform even the most rudimentary of tasks．"
And Scotland's largest city, Glasgow, was shamed as the most inactive city in the UK, with 75% surveyed admitting they do not get enough exercise, followed closely by Birmingham and Southampton, both with 67%．
The results cause serious challenges for the National Health Service, where obesity-related illnesses such as heart disease and cancer have been on a steady increase for the past 40 years and are costing billions of pounds every year．
